**ALL student-athletes are required to submit a photocopy (front-and-back) of their current medical insurance card along with the necessary forms. This is in addition to the insurance information that is submitted to the Cabrini College Health Services Office.**

**ANY student-athlete that fails to submit this to the Athletic Training Staff may be withheld from participating in team practices and/or contests until this information is received.**
